It is possible to promote members to captain if you use the upgrade url again after it disappears. Happened to me when I used control click when trying to upgrade a user.

If this happened while trying to promote me, it was not a bug but the game recognizing my greatness.
Of course that was the case, Taser. When I tried to promote you, you instantly became Leader so I had to kick you from the House to prevent treason x[
