Have only seen this bug a few times but maybe it could be an easy fix
I understand why it happens and with that being a very rare situation i dont think we will see a fix.

another bug concerning tomes: sometimes one will only spawn with 1 instead of 2

That's a lot more of a common problem and you can even do the opposite with it being 1 more than normal.
