I assume it was when I had been using a Huscarl's Shield and equipped a Champion Huscarl's Shield, or vice versa.
Dropping and re-equipping the shield fixed the problem.
(I am able to wield 2 shields at the same time.
)Another problem is the bugged texture of Nord Cavalries shields, but I reckon that has been noticed by devs already.

edit: Its perfectly reproducable, just wield any shield together with medic boxes and grab one shield from ground and it will be used as a weapon.
