(While only having one copy of the sword) When the sword is drawn, the sheathed version of it remains, so you have one in your hand, and one on your side.

Are you spawning in two of them? This same thing happened to me with a different sword when I equipped multiple.

No, only the one; it seems to be being treated as a sheath, rather than the weapon inside the sheath, so it stays on the side when you draw the weapon.
