I'm happy that you two decide to merge your posts; working together on something could help you two learn much faster.
Rydell:
The polehammer model is good, but I have some concerns. The UV unwrap is stretched in some areas, like the merging of the head of the hammer to the shaft, or the leather grips. The model itself also is not "smooth". Soft edges and hard edges are very important with low-poly models because it helps convey certain areas over others, but your model has all hard edges, which means it ends up looking blocky. Try smoothing the whole model and then dictating the sharp edges yourself, as it lets you gain control of which parts you want smooth or not.
Zocki:
Your sword is coming along nicely. It has improved very much since you've updated it, but it still needs some work on the blade. The blade is still very thick; it's not supposed to be as thick as the handle or the crossguard. The blade is the thinnest part of the sword (most of the time), and to reflect that you'd need to scale it thinner. I've made a mock-up of what that would look like:

After doing this, you still need to do some work on the tip of the blade. As the blade turns to a point, you need to scale it down to reflect the sword edge scaling down to a point as well, like so:
